RSC-TCM provides specialized support to eligible members currently living in qualified institutions who desire to transition into the community. Our mission is to ensure a well-coordinated and seamless transition process by connecting individuals with vital services such as medical, social, educational, and housing resources that are specifically tailored to meet their individual needs and circumstances.

Our goal is to empower each member to successfully integrate into community life by offering comprehensive assistance and guidance every step of the way. We are committed to facilitating a smooth and planned transition that respects the personal choices and aspirations of our clients, ultimately fostering independence and a positive quality of life in their new community setting.

Our Approach

    Support with housing application processes, even for those with a challenging rental history.

    Coordination of tours to prospective relocation sites.

    Assisting individuals in applying for various income sources, including general assistance and social security benefits.

    Facilitating the eligibility process of waiver services through the State Medical Review Team (SMRT) or Social Security Administration (SSA).

  • Additional Support

    Contacting relevant county agencies and initiating the relocation process.

    Assistance with record expungement advocacy.

    Coordination of essential services such as movers, household items, and furniture.

    Arrangement of transportation services.
