
MOVING HOME MINNESOTA
Moving Home Minnesota (MHM) is a federal initiative aimed at empowering individuals in Minnesota by facilitating their transition from institutional settings to independent living within the community. MHM focuses on person-centered planning and delivering services in settings that offer the least restriction and the highest degree of integration possible.
The project is designed to support individuals in achieving their goals of living independently and participating fully in community life. By emphasizing personalized planning, MHM ensures that services are tailored to meet the unique needs and preferences of each participant. This approach not only promotes autonomy but also enhances the overall quality of life for individuals transitioning from institutions to community-based living arrangements. MHM is committed to fostering a supportive environment where individuals can thrive and contribute meaningfully to their communities.
Eligibility Criteria
To qualify for MHM services, individuals must meet specific eligibility criteria, including:
Residence in a Qualified Institution: The individual must currently reside in a qualified institution and have continuously lived in one or more qualified institutions for 60 or more consecutive days.
Enrollment in Medical Assistance (MA): The individual must be enrolled in MA prior to service initiation through discharge from the qualified institution and maintain this enrollment throughout their eligibility for MHM services.
Transition to a Qualified Community Residence: Upon leaving the institution, the individual must transition to a qualified community residence and continue to meet the institutional level of care criteria to maintain eligibility for MHM community-based services.
Qualified Institutions
MHM services are available to individuals residing in various qualified institutions, including:
Community Behavioral Health Hospitals (CBHH)
Hospitals
Institutions for Mental Diseases (IMD), such as Anoka Metro Regional Treatment Center
IMD facilities enrolled in the 1115 Substance Use Disorder (SUD) System Reform Demonstration
Intermediate Care Facilities for Persons with Developmental Disabilities (ICF/DD)
Nursing facilities
Psychiatric Residential Treatment Facilities (PRTF) for children and adolescents
Qualified Community Residences
To maintain eligibility for MHM services, individuals must transition to a qualified community residence, which may include:
A home owned or leased by the member or their family member
An apartment with an individual lease, where the member or their family has domain and control
An assisted-living residence with separate living, sleeping, bathing, and cooking areas and lockable entrance and exit doors
A residential home where no more than four unrelated individuals reside
